How do I embed tables into forms?

You can create the table in Microsoft Word. Once created, simply copy and paste into your form.

Can I automate key data from existing employee record?

The key data such as name, current employment and deployment can be found in the report for the form, it's not currently possible to bring data from the record on to the form.
