A Question and Answer section on an appraisal form enables a list of questions to be created that employees must answer as part of the process.
The answers can be a text field or multiple choice.
To use multiple choice questions, one of the following rating scales must be set up:
Adding a Question and Answer Section
Section Details
- Enter a Section Name. (This will be displayed as a heading on the appraisal form)
- (Optional) Enter a Section Introduction. (This will be displayed directly below the Section Name on the appraisal form)
- (Optional) Choose whether to Allow Linking to Development Plan.
- Select the section Sort Order.

Question Settings
- (Optional) Choose whether to Allow Question Editing. (If selected, participants will be able to add, edit or delete questions from the form)
- Select Manage to add questions to the form.

Manage Questions
- Select Add New.
- Enter the Question Text - this is how the question will be displayed on the appraisal form.
- Select the Question Type - this is how the question will be answered.
- If the question is Multiple Choice, select a Rating Scale.
- If the question is Text, select whether to use a Long Text field.
- Select the question Sort Order.
