Question and Answer Section

A Question and Answer section on an appraisal form enables a list of questions to be created that employees must answer as part of the process.

The answers can be a text field or multiple choice.

Note:

  • Different rating scales can be chosen for different questions (meaning that you don't have to have the same available options throughout all of the questions asked).

To use multiple choice questions, one of the following rating scales must be set up:

Adding a Question and Answer Section

Section Details

  1. Enter a Section Name. (This will be displayed as a heading on the appraisal form)
  2. (Optional) Enter a Section Introduction. (This will be displayed directly below the Section Name on the appraisal form)
  3. (Optional) Choose whether to Allow Linking to Development Plan.
  4. Select the section Sort Order.

Question Settings

  1. (Optional) Choose whether to Allow Question Editing. (If selected, participants will be able to add, edit or delete questions from the form)
  2. Select Manage to add questions to the form.

Manage Questions

  1. Select Add New.
  2. Enter the Question Text - this is how the question will be displayed on the appraisal form.
  3. Select the Question Type - this is how the question will be answered.
  4. If the question is Multiple Choice, select a Rating Scale.
  5. If the question is Text, select whether to use a Long Text field.
  6. Select the question Sort Order.
